Here is what the form looks like, I have circled the initial code action field and the print output action field.  In addition, if you want to create the QR code with a transparent background, his online form will do that as well when you select the box next to the word “Transparent”   in the background color field.

And the form’s interface will change depending on the type of QR Code you want to generate.  For my purposes, I wanted to see what size the QR code is generated in and the file output options it provided, those choices are .png, .pdf, .tiff, .svg and .eps.

He also includes in his instructions a notable point for graphic designers and others who want to print their QR Codes and that is  “when codes are generated, they are created in RGB …and if they are used in printing, they should be converted to grayscale or CMYK so that, when printed, only the black plate is used. This ensures the highest edge resolution, particularly in smaller sizes.”
